Event Request Form

A large volume of invitations to events and requests for meetings are received by the Mayor's Office.  As a result, requestors are asked to adhere to the City's established guidelines.

In order to invite Mayor Frank G. Jackson to participate in an event or program, interested parties must complete the Event Request Form. 

This form may be submitted either electronically to rjdavis@city.cleveland.oh.us or a hard copy can be sent via fax to 216.420.8758.  All forms must be completed and returned at least 30 days prior to the event.
